Once you are directed to the FORMED website, follow the below steps to
sign up for free through our Parish:

1. Sign up as a parishioner

2. Type in "St. Alphonsus Church Maple Glen."
Select our Parish, which should be the top choice given, then click "Next."

3. Enter your name and your email then click "Sign Up."


If you've already made an account with FORMED through
the Parish then simply click sign in. This will prompt you for your email
which they will send a link to in order to sign in.
